Building a Statewide Clinically Integrated Delivery System

Many state Medicaid programs are on a trajectory of insolvency and searching for solutions among multiple healthcare reform strategies, including value-based care. In order to succeed in a value-based system, many healthcare organizations are building collaborative networks to coordinate patient care for shared populations. By working together to develop best practices and standardized ways of practicing medicine, patient health outcomes can be improved, quality can be enhanced, and costs can be reduced. This presentation explores an innovative, statewide approach to developing a clinically integrated delivery system known as the Illinois Partnership for Health by reviewing the key components of the model, discussing the operational challenges, and sharing best practices.
